On or after 1st Birthday	Measles, Mumps, Rubella (MMR) Meningococcal C (Menjugate) Pneumococcal Conjugate (Prevnar) Varicella (Chicken pox) if susceptible
18 Months	Diphtheria/Tetanus/Acellular Pertussis/IPV/Hib (Pentacel) Measles, Mumps, Rubella (MMR) Pneumococcal conjugate (only for those infants who have not received a pneumococcal conjugate vaccine at greater than or equal to 12 months old.)
School Entry (4-6 years)	Diphtheria/Tetanus/Acellular Pertussis/IPV (Quadracel) Varicella (chicken pox vaccine) if susceptible
